 How to make an appointment?

If you would like to schedule an appointment in Dublin please call and speak to Owen on 01 8211796 between 12 and 3 pm, Monday to Friday.

GP Referral Letter, Questionnaire and Consent Form must be completed and brought to your appointment.

The questionnaire (pages 1-7) MUST be completed by relevant Education Providers and the last page (page 2) MUST be completed by all legal parent(s) and Guardian(s) prior to attendance.

What to bring?

Bring the aforesaid document above, a hard copy of relevant professional reports including school reports and details of specific care needs.


It’s helpful if both parents can attend but not essential

(Beside Lloyds Pharmacy and across the road from Supervalu)

Parking is free on the main street and there is a pay and display car park at the back of the clinic. Beware of clamping.

Directions from the M50

Take the Blanchardstown exit, M3, on the M50 Direction Cavan,

Follow the signs for Blancharstown Village approx 5 minutes drive from the M50 on the left you will see a sign the Blancharstown Village, follow this sign

This takes you onto the Navan Road, follow the street along until it turns into  Main Street

The clinic is on right hand side of the main street just after Mill Road.

Stop just before Supervalu on the left and Lloyds Pharmacy on the right hand side

The clinic is beside Lloyds Pharmacy and parking is free in the main street

There is a pay and display car park at the back of the clinic

When do I get the report?

Reports are usually available in one to two weeks.
